About Transbay Medical Group

Our mission is to help seniors maintain their physical and cognitive abilities as they age. We also provide training and close oversight so facilities can better meet the needs of their residents.

 

Transbay Medical Group is a small, independent practice specializing in post-acute medical care and outpatient primary care for elders living in resident care facilities for the elderly (RCFEs).

Since 2017, we’ve offered a full spectrum of care helping seniors maintain a good quality of life throughout the aging process. We also help patients recover after hospitalizations, as well as transition to palliative or hospice care when and if that time comes.

Samir Sairam, MD • Founder

Dr. Sairam has been a practicing physician in the Bay Area since 2008 and started his career as a hospitalist with the Sutter East Bay Medical Foundation. He worked as a full-time hospitalist there until 2014, and he remains on staff at AltaBates-Summit Medical Center in a part-time capacity. Dr. Sairam transitioned to working with elders in post-acute care in 2015 and has served as a medical director at multiple skilled nursing facilities and RCFEs in the East Bay.

Dr. Sairam grew up in the Bay Area and graduated from the University of California San Diego in 1997 with a B.S in Animal Physiology and Neuroscience.
He went on to complete his Medical Degree from the American University of the Caribbean (St. Maarten, NA) in 2003, returning to California for his residency training in Family and Community Medicine at UCSF-Fresno. He completed his residency training and board certification in Family Medicine in 2007. He is a member of the American Association of Family Physicians (AAFP), Society of Hospitalist Medicine (SHM), and American Medical Directors Association (AMDA).

Outside of his practice, Dr. Sairam enjoys hiking, camping, snowboarding, tennis, and cycling. He’s got a passion for modern architecture, cooking, and photography, and he is learning to play the ukulele.

Nicole Long, MS, FNP

Nicole is a nationally certified family nurse practitioner who has been working with adults in skilled nursing, assisted living and primary care for more than 12 years. Her main goal is to provide care that keeps patients healthy and strong, and she enjoys teaching her patients how to prevent poor health outcomes and continue a high quality of life no matter their age.

Nicole has been a registered nurse for more than 20 years and a nurse practitioner for more than 17. She has a bachelor of science degree in physiological science from University of California, Los Angeles (UCLA) and Masters of Science in Nursing from California State University in San Francisco. She received her nurse practitioner certification from California State University in San Jose. Nicole belongs to the American Association of Nurse Practitioners and the California Association of Nurse Practitioners and volunteers locally with Scouts BSA and Cub Scouts. As a mom of three, she remains calm through yoga, gardening, raising chickens, and walking in nature.
