Because we accept insurance from all major providers*—Medicare, HMO / PPO, Managed Medicare plans, and Accredited Care Organizations—you’re able to get treatment and services from us just as you do from your current primary care physician; no extra fees.

Here’s how we can help:

•You can get direct, on-site primary care services right from the comfort of your retirement or rehab facility.

•We also provide adjunct care for patients who have been admitted to a skilled nursing care facility after hospitalization.

•Want to keep your favorite primary care doctor? No problem. We’ll work closely with them to ensure complete continuity of care.

•We offer full telehealth services, meaning we can do appointments via video if desired. We encourage family participation in these video visits!

•You’ll be seen regularly by our staff, with balanced care from both our MD and our Nurse Practitioner.

•Need specialist care? We’re able to refer to highly regarded specialists in the community, including those practicing with Sutter Regional Network, Stanford Healthcare, and UCSF Healthcare systems.

•You can contact us by phone and email 24/7.

•Should you need help making end-of-life decisions, we’re here to help. We’ll provide clear answers to difficult family questions and work closely with hospice agencies to gracefully co-manage our patients who are in their care.

 

*Unfortunately we cannot treat patients who are covered by Kaiser Permanente.
