How do I get an appointment with you?

We work closely with the nursing staff and medical assistants at our facilities to coordinate future visits with our patients. If you have a need to be seen, we ask that you first alert the nurses at your home facility and they will schedule a subsequent visit with us. This allows the nurses who provide care for our patients to be included in the ongoing service to our patients.

Can you see me in person?

Yes, we can see patients in person. Due to the Covid-19 pandemic, we have minimized our direct inpatient care; however, with the recent availability of vaccinations for elders and for the staff that care for our patients, we have started to once again see patients in person.

Are you able to see patients via telehealth?

Yes, we can provide care via telehealth medicine either over the phone or through video calls.

Can I see you on an as-needed basis and continue my relationship with my primary care providers?

Yes, you can see us on an as-needed basis, much like you would seek care at an urgent care clinic. We can provide this service for all insurance plans except Kaiser Permanente.

How do I contact you?

Patients and administrators can leave a message on our practice telephone number. We also encourage families to use this number if they have any questions or concerns about their family members who are under our care.

How do I access my medical records?

You can access your medical records through the medical records department of the facility where you reside. If you have signed up for the patient portal access to our EMR system, you can also access your records there.

Can you provide me with referrals to specialists?

Yes, we can provide you with referrals to all necessary specialists at local institutions in the area, including UCSF, Stanford, and Sutter.

What happens if I get hospitalized? Can I still get care from you?

We do not see our patients while they are hospitalized, but we work closely with the ER physicians and hospitalist groups to coordinate hospital admission and discharges. We also see our hospitalized patients after discharge in skilled nursing and through outpatient visits.


Do you have an office?

We see our patients in their home facilities; we do not have a medical office. We see patients across all settings, including independent living, assisted living, memory care, and skilled nursing care facilities. Some of our facility partners have in-house clinics, and we can see patients at these locations as outpatient visits. We also provide in-home visitation on an as-needed basis.

Do you provide concierge care?

At this time, we do not provide concierge medicine. We use a primary care model for our patients, with direct fee for service through all insurance plans.

Do you see Kaiser patients?

We do not provide direct patient care for Kaiser patients. And while we do provide guidance in our facilities as needed for the management of a Kaiser patient on occasion, we do defer to Kaiser primary care providers for their medical care.

Can you help manage our medications and refill prescriptions?

We are able to electronically fill and refill medications at both local and mail-order pharmacies; most will deliver to your door.
